The Ambulatory Care Suite includes a capability that fundamentally changes what ambulatory care looks like between visits. We call it the Living Care Plan.
In legacy ambulatory systems, the patient portal is retrospective. You log in to see what happened at your last visit. Past results. Past notes. Past medications. The portal is a record of care that already happened.
In MyNursePal Pro, the portal is the care itself, continuing. After the visit, the patient's care goals stay active — A1c targets, blood pressure goals, medication titration plans. Abby, the AI care companion built into the platform, coaches the patient between visits: medication reminders with context, behavior nudges, check-ins on the goals their physician set.
The clinical team sees what Abby sees. Medication adherence is tracked, not assumed. Behavior patterns surface over time. At the next visit, the physician knows whether stalled outcomes are an adherence problem or a medication problem — and can prescribe accordingly, instead of guessing.
